PRINCIPALS OF PHOTODYNAMIC THERAPY04

The basis of PDT is the interaction of light with photosensitive agents to produce an energy transfer and a local chemical effect. This is broadly similar to what is seen in photosynthesis, although in this case, many photosensitizers work together to harvest light energy to produce chemical reactions. Usually, the photosensitizer is excited from a ground singlet state to an excited singlet state. It then undergoes intersystem crossing to a longer-lived excited triplet state. One of the few chemical species present in tissue with a ground triplet state is molecular oxygen. When the photosensitizer and an oxygen molecule are in proximity, an energy transfer can take place that allows the photosensitizer to relax to its ground singlet state, and create an excited singlet state oxygen molecule. Singlet oxygen is a very aggressive chemical species and will very rapidly react with any nearby biomolecules. Ultimately, these destructive reactions will kill cells through apoptosis or necrosis. PDT can be considered a form of targeted singlet oxygen chemotherapy, where the targeting is achieved with the combination of the photosensitizer (functioning as a catalyst) and intense light.

Principle photomodulation:

Stimulation of mitochondria by trains of photons emitted by the LEDs, results in a transformation of adenosine diphosphate into adenosine triphosphate to recharge the battery cell and promote the energy for growth purposes (production neocollagen). This principle is very close to the photo-modulation principle of photosynthesis. The Medelisol lamp emitting in the orange (590nm + 630nm) – close to the absorption of cytochrome – is best suited to the production of neocollagen photo-modulation.

In my office, we use LED lights at a rate of one to two sessions of half an hour per week for 5 weeks. The LEDs will densify the dermis (facial rejuvenation, neck, ..), reduce the inflammation (redness reduce scarring) and improve a little hyperpigmentation disorders disappear without lentigines (age spots) already present. Finally, a few LED lights or sessions can be performed in patients to the “wait” or conversely to perfect an outcome in a patient that we would “reward”.

The results start to be visible after the third session and the optimum is obtained three months after the end of treatment. “
